Qualification procedure and booking a date for the vasectomy

- Straight after the date has been agreed, fill in the health questionnaire. The form works dynamically and is shared by both our locations – Warsaw and Katowice.
- To the e-mail address you provide we will send a confirmation of the date together with documents to read: the text of the informed consent and detailed information about the procedure.
- A few days before the planned date, the doctor performing the procedure will contact you by phone. If any additional information or tests are needed, he will discuss them with you at that stage.
The telephone conversation and the health questionnaire are part of the medical interview and form an integral element of the qualification process.
